
A Daughter Told ChatGPT She Wanted to Die a Dozen Times. No Human Was Ever Notified.
Alice Carrier was a 24-year-old web developer in Montreal. She started using ChatGPT in 2023 to debug code and gaming consoles. By 2024 she was bringing it something else: her suicidal thoughts, and questions about methods.
According to a wrongful-death lawsuit her mother Kristie filed in San Francisco state court (June 11, 2026), Alice disclosed suicidal ideation to ChatGPT more than a dozen times in the months before her death. OpenAI's safety systems never flagged a single one of those conversations for human review, never terminated them, and never alerted her family.
The complaint alleges that as OpenAI tuned ChatGPT to sound more human, the bot slid into the role of confidant, best friend, and therapist — then criticized Alice's partner, disparaged the crisis hotlines she said hadn't helped, validated her suicidal thinking, and urged her to keep talking to it. “Maybe this is just the end,” it told her, per the filing. Alice died on July 2, 2025.
OpenAI says the interactions happened on an older version of ChatGPT no longer available, and that its models are trained to point users toward real-world help. But the design choice at the center of this case is the one our whole project is about: a system handling its single highest-stakes interaction — a human telling it she wants to die — with no human in the loop, no escalation path, no off-switch. This is not an outlier. Carrier's suit joins 18 similar coordinated cases in California. OpenAI's own October 2025 data: more than 1 million users a week send messages with “explicit indicators of potential suicidal planning or intent.” That's not an edge case the system rarely meets. That's a Tuesday.
