Asked to Delete One Folder, the Agent Took the Whole Drive: Two Codex Users Report Catastrophic File Loss

Two developers filed reports on OpenAI's public Codex issue tracker on October 26 and 27 describing catastrophic file deletions by the AI coding agent.

In the first, a user says a malformed Windows command meant to remove a single node_modules folder instead ran across an entire SSD partition, deleting photographs, university work, unrelated projects and Git history, some of it likely unrecoverable. The report includes the exact command, timestamps and output.

In the second, a user says the agent created a Windows junction inside a Unity project's temp folder during testing, after which 4,864 tracked source assets and uncommitted work were found deleted.

These are user reports, not confirmed findings: OpenAI has not publicly verified either account or identified a root cause. But together they point to the same missing safeguard. An agent with full file access needs a hard boundary that confines it to the project and stops any deletion outside that boundary for human confirmation, no matter what the user enabled.

Why this matters to youNo jargon — just what it means

Picture asking a helper to throw out one old shoebox from your closet. You come back and the whole closet is empty: your photos, your school papers, your projects, all of it. That is what one developer says happened when an AI coding helper was told to delete a single folder. One tiny mistake in one command, and years of someone's life were gone in about 19 seconds. A second user says the same kind of helper wiped 4,864 project files during a test. That person got their files back after a lot of recovery work. The first person did not get back their photos.

Here's why that's a big deal: these helpers are fast, and fast is great until it is fast in the wrong direction. Nothing stopped the helper and asked, "Are you sure? This is outside your project." Until there is a wall the helper can't cross without a human saying yes, one typo can delete a life's worth of files.
